Comes now the District Attorney of Dallas County, Texas, by and through the undersigned Assistant District 
Attorney, and in the above cause would show that Defendant stands charged with a felony offense, and respectfully 
requests the Court to reduce the offense charged to the lesser and included Misdemeanor offense of:

Comes now Defendant in the above cause, and would show that he/she now stands charged in the above cause 
with a  Misdemeanor Offense, states that he/she will enter his/her plea of GUILTY to said charge at the proper time 
in open Court, and with his/her attorney being present, and hereby waives his/her right to trial by jury and asks the 
Court to proceed to trial herein without a jury and to consent to Defendant's waiver of trial by jury.  
  
Defendant further prays that the Court accept his/her plea of GUILTY and proceed to enter Judgment herein as 
provided by law and Defendant requests that the Court place him/her on probation in this cause.  
  
The foregoing Wavier of trial by jury, made by the Defendant in open Court and in the presence of his/her Attorney,
